Tower running: getting to the top of the Gherkin ? the hard way

Lactic acid, burning lungs and a stunning view as hundreds of runners race to the top of the great London skyscraper

Before the Shard, the Cheesegrater and the Walkie Talkie was the Gherkin ? and the hundreds of runners who raced up 1,000 steps to the exclusive private members' bar at the top of Norman Foster's iconic skyscraper at the weekend got to enjoy stunning 360-degree views of London ? and raise funds for the NSPCC.

The Gherkin Challenge may be over quickly but the lactic-acid burn from racing up 180 vertical metres means it should not be underestimated. "The toughness comes from the vertical gain and, in particular, the second half," warned running coach Martin Yelling before the event. "Start the steps too quickly and the final few hundred will be leg-sappingly uncomfortable."

With his words in mind, I paced myself slowly at the start ? striding two steps at a time and using the stair rail to haul myself higher. Spiralling up 38 floors in a tight stairwell is kind of hypnotic ? before I knew it, I was past the 20th floor and although I could really feel the burn in my thighs, I knew I wouldn't be collapsing in a breathless heap on the floor. A couple of minutes later I was past floor 30 and tried to kick ? but quickly realised I didn't have anything left.

The breathtaking panorama from under the glass dome (the only bit of curved glass in the building, apparently, despite its shape), a complimentary glass of champagne and, of course, doing it for a good cause made it all worthwhile ? and, given the lengthy queues to get inside 30 St Mary Axe at last year's Open House weekend, the Gherkin Challenge may be one of the best ways to experience Foster's masterpiece from the inside.

Elite stair runner Matthias Jahn of Germany set the fastest time with 4 minutes 18 seconds ? a new record for the event.

Britain is unusual in that its few tower-running events are all held in aid of charity. In other countries it is a competitive sport, with elite runners racing up skyscrapers including the Empire State Building in New York and the 2,046 steps of Taipei 101 in Taiwan. And while it may not take place inside a tower, the 11,674 steps of the Niesen railway staircase race in Switzerland takes some beating.

Michael Reichetzeder, executive director of the Tower-running World Council, said: "Why do I do this? This question comes to mind for nearly everyone competing in a stair race at some time during the event. But once you reach the top, the world looks different. You have fulfilled an inner human desire to reach for the top. Generally the exhaustion does not last long, and often you are rewarded by a grand view."

For 2013, the NSPCC expanded the Gherkin Challenge to run over two days, with an Everest Basecamp-themed family festival on Saturday and more competitive individuals and teams taking part on Sunday ? although the charity stresses the event isn't about fast times, with finishers reaching the summit in anywhere between four-and-a-half and 45 minutes.

The NSPCC hopes the 550 runners this year raised 150,000 to help protect vulnerable children through its projects and services, including the free, confidential 24-hour ChildLine helpline and online service.

Even shorter bouts of high-intensity exercise improve physical fitness in inactive men

It is a commonly held perception that getting in shape and staying there requires hard work and hours upon hours of training. Now, new research shows the opposite ? just four minutes of vigorous activity three times per week is enough to make previously inactive men fitter and more healthy.

How I got hooked on open-water swimming

After being in Lake Windermere with 10,000 others, I can understand why competitive open-water swimming is so popular

I have only swum in open water a few times, and always in calm reservoirs or gentle lakes, so I wasn't quite prepared for how imposing and rough Lake Windermere appeared on a cold grey day. But I, along with 10,000 others, braved the lake to complete the challenge of the Great North Swim, an experience I am already keen to repeat.

The Great North Swim, and open-water swimming in general, is seeing a surge in popularity. The event started in 2008 with 3,000 participants, and now 10,000 take the plunge, going from their local pools to the breathtaking setting of Lake Windermere. It's part of a series of five swims throughout the country, with 20,000 people swapping lanes and chlorine for the great outdoors.

Simon Griffiths of H2Open magazine says he has noticed an increased interest in the sport in the last few years. He believes that becoming an Olympic event helped raise its profile, while the growth in triathlon means stand-alone swimming events are often run alongside triathlons. There are now about 200 swim events each year in the UK.

I arrive on a train packed with people descending on Windermere, and the hotel is buzzing with talk about what distances they are doing, and swapping techniques and advice. I head straight to the lake just as the 5km race is finishing, the longest distance over the weekend. David Hastie, who has just completed this race, tells me the water feels colder than the 15.6C it's been measured at. "It's much nicer than swimming in a pool," he says. "You just have to focus. It's very choppy though."

Choppy seems to be the description most people repeat as they emerge from the water. Even Christine Jennings, who won the Women's Elite race in 19:05, uses that word. But if 10,000 other people aren't daunted, I won't be either.

My training has involved a few sessions in a local reservoir, but has mainly been in a pool where sighting is fairly straightforward. It's very different in a choppy lake. My new Huub wetsuit only arrived two days before the swim so I popped to my local lido to give it a quick tryout. I managed to do the mile there in 29:30 ? but how would I fare in open water?

Most of the people taking part are doing the one-mile race, of which there are 26 waves over the weekend. There seems to be a mix of open-water enthusiasts alongside complete novices ? which is precisely the aim of the swims, to get as many people as possible completing their own challenge. The oldest woman competing is 77, taking part in the two-mile race alongside a man who last year swam in every mile swim wave.

I have opted for the mile in the third wave of the day, which gives me time to watch the impressive performance from the elites and the start of the masses. Former Olympic butterfly swimmer Stephen Parry emerges from his mile event, completing it in 22:29, and I ask his advice for my race. He tells me: "You'll be fine, it's all about sighting and staying calm." Easy for him to say.

There are just over 200 people in my wave. We are all taken through an acclimatisation area, a paddling pool-sized part of the lake where we plunge in to feel how cold the water is. Not too bad is the verdict! Iwan Thomas, the Great Swim ambassador and sprinter turned open-water swimmer, is going to start our race before taking part in the next wave. He tells us all to relax and enjoy it, though he seems more nervous than the rest of us.

We take off with a flurry of splashes and head out towards the middle of the lake. We were warned that the first 100m would be really choppy, but that it would feel much calmer after that. Somewhere near the 750m mark I'm still waiting for the calm, and at times it felt more like swimming in the sea than a lake.

I feel battered by the water. I try to focus on my breathing and technique, and just keep going. Somewhere near the 400m-to-go mark my lower right leg cramps painfully. I recall overhearing people in the bar the night before talking about how they keep swimming through cramp, so I try. It doesn't work, so now instead of just my lower calf cramping the entire leg goes into spasm. I don't want to stop, so flex my foot and just kick with the left leg: fine until a friendly steward in a kayak points out that I've drifted off course. However, I can now see the finishing post so just concentrate on getting there ? still one-legged.

My finishing time is 38:23 but that doesn't seem to matter ? the atmosphere is fantastic and everyone feels a sense of achievement, whatever their time. Ten thousand people can't be wrong. I'm hooked, and want to give it another go. I've already signed up for the Great Swim in London.

Lizzy Hawker: 'I might run around 180 miles in a week'

Five-time winner of the 103-mile Ultra-Trail du Mont-Blanc and former 24-hour road running world record holder Lizzy Hawker has been called Britain's greatest endurance athlete. She talks to Nick Mead about how ultra distance running is a way of life - and how Tibetan biscuits are a good fuel

Hi Lizzy. When did you start running? We were all runners as children, so I can't really remember not running. I never raced or joined a club when I was younger. It just became normal by some stage to run every day ? it's my way of being outside and moving. It wasn't until I was working at the British Antarctic Survey that I ran my first marathon ? the London marathon ? I think I tried a couple of times to get in through the lottery and didn't, but finally in 2000 I did.

Do you remember how you got on? I'd been at sea for six weeks until not long before the marathon, so I didn't have the ideal training conditions. I loved the experience but my time was nothing special ? around 3hrs 40min ? I didn't really think of doing much more. Then I remember a friend suggesting that if I like mountains and I like running, why don't I do a road marathon on a hilly route. So I did the Snowdonia Marathon a few times. And then I thought, rather than just running around the mountains, I'll try running on them.

The Welsh 1000m Peaks was my first non-road race ? but I didn't have any fell-running experience, so I couldn't enter the running class, and had to do the mountaineers' class ? so that was big walking boots, long trousers and a rucksack. [Lizzy won the women's race]

How did you get into ultra-distance running? It's really in 2005 that I fell into long distance ? both in the mountains and on the road. I was going to visit friends I'd met doing the Welsh 1000m Peaks during my PhD [in physical oceanography] and they happened to be doing a 40-mile track race ? because I was going to be there for the weekend I thought I might as well try it. The England selector was there and asked if I'd represent Britain at the UK 100km championships, a road race, which was just a month later.

I'd read an article about the UTMB and entered it. At the time the event was quite new and you didn't have to do qualifying races like you do now. I was due to finish my studies that summer and thought that if I entered I could go out to the Alps for a couple of weeks, do some climbing and do the race at the end of it. So it started by chance.

And the rest is history, as they say ? five times winner of the UTMB, 24-hour road running world record holder ? you've been spectacularly successful since then. I doesn't feel like that to me. I don't feel like I've done anything special and I still feel like there's more potential to dig out. For me it just feels normal.

When you did the UTMB for the first time in 2005, had you trained specifically for that? Not really, because I had no idea what was involved. When I was standing on the start line I really had no concept. I mean, I knew the figures and things [103 miles of trail through the French, Italian and Swiss alps, including 31,168ft of ascent] ? but I had no idea if I could get to the finish line, or if they'd tell me to stop somewhere along the way.

There was no specific training that I'd done ? but I'd been hiking in the Alps already that summer and I'd had a trip to Skye. For a race like the UTMB just being out for long hours on your feet is half the battle and I did have that endurance behind me.

When you're training now, what's your typical weekly mileage? During the winter sometimes I try to slip into more of a typical marathon-style training. Then in the summer I tend to run more. Last summer there was a week when I did a 70-mile mountain race, and then I was working on a running camp, so maybe another 110 miles in four days around the Tour de Monte Rosa, and then I raced a short mountain race ? so that week probably got up to around 180 miles. That might happen a couple of times during the summer.

What's the furthest you've ever run in a race? In a race, it's probably Spartathlon, which is 155 miles, and that's more or less the distance I ran during a 24-hour road race [Lizzy set the 24-hr road running world record of 153.5 miles in Llandudno in 2011].

But I've just come back from a stage race in Mustang, Nepal, and before that I'd been up to Everest base camp, and run back to Kathmandu, which is something I've done a couple of times before, and managed to break my record this time. That's 199 miles, which I did in 63 hours.

That's incredible. Were you running the whole time ? or did you grab some short naps? I maybe stopped twice for about 20 minutes to lie down ? but I didn't actually sleep. Then in the last 10 miles, coming back into Kathmandu, darkness fell on the third night ? and that really hits you. I remember just lying at the side of the road for two minutes. I'm not sure you can call it sleep, but I went somewhere.

What do you think makes you good at running ultras? Have you got something different ? or have you managed to connect to something everybody has inside them? For me, endurance just comes naturally. It's always been a way of life ? from before I ever got into racing and long-distance running. Even when I was a child I preferred to walk than to take the bus ? to cycle rather than drive. That's always been there. That's probably different for most people. With ultra and endurance sports the mental side is really very important, and that's also part of who I am. I guess we're all looking for something in life ? I find something through running.

What's the best thing about running for you? I think I just like moving ? but under my own power ? and of course I love the mountains. The love of running is a little bit separate from that because I'll run wherever I am, whether it's pavement or trail. Maybe it's the physical movement ? and the mental freedom.

If someone asks you for a training tip, what do you say?
With long distances, it's really about staying in the moment. If you can do that, and have the confidence to try a long run, then our limits are never where we think they are. You realise you can go beyond what you thought was possible for you.

When you're running these enormous distances you must hit 'the wall' numerous times. How do you deal with that? I think it's matter of knowing that there will be times when you feel great and there will be times where you feel really, really not great. It's one of those truths about life ? that nothing lasts, everything is impermanent ? so it's just knowing that those bad patches are going to pass and you'll come through the other side and trusting in that.

If you stand on the start line of a 24-hour race and think about how long you'll be running it's almost inconceivable. You have to take it moment by moment. It's the same in a 100-mile race ? if you think about the finish at the start then you're probably not going to make it ? but if you take it step by step, stage by stage, then you realise it is actually possible to run that far. If I have a race strategy it's just to run the best that I can at any point in the race.

How about nutrition. What do you eat before a long run, for example, and what would you eat during a run? For breakfast, usually bread and honey and milky coffee. When I'm running I eat real food ? I don't use gels and sports drinks. It'll be a mixture of stuff, depending on where I am and what's available. In the Mustang, for example, you get these fantastic biscuits that are made from Tsampa, roasted barley, that are just a little bit sweet ? it's a staple food there and they're very easy to carry.

And what's your favourite post-race indulgence? The evening after a race a glass of wine is very nice. But if you've been running for 24 hours or longer it can take a while for your stomach to settle down, so it might be a few hours before you feel like eating or drinking much. And it depends on where you are, what's available ?

Do you use any running gadgets? I'm low-tech ? as minimal as you can get. I do have a GPS now which I'll use if I want to make a certain distance in training but I do most of my running without it, and I don't watch my heart rate or anything like that.

Finally, who do you think is the greatest runner of all time? That's too hard. I'll duck out and say something general, like the person who inspired you to run. It could be anyone ? seeing other people out running can inspire you to put your trainers on and get out there.

Bloodbot, The Bright Future of Phlebotomy?

Here is a project from the folks at the Department of Mechanical Engineering at Imperial College London, and its Mechatronics in Medicine Laboratory, who are bent on developing an "active robot designed to take blood samples from the ante-cubital fossa..."

The Bloodbot identifies the location of a suitable vein by pressing a probe against the surface tissue of the ante-cubital fossa and measuring the force on the probe. The difference in the characteristics of the tissue from its surroundings, in response to the applied force, indicates the presence of a vein.

Once a suitable vein has been found, it inserts a needle under force control. When the needle penetrates the vein (identified by its force/position profile), the control system prevents further insertion, thus avoiding overshootin...

Become a marathon runner with the protein PGC-1alpha

Even with a greater muscle mass, a sprinter cannot win a marathon. His specially-trained and strengthened muscles will fatigue faster than the endurance-trained muscles of a long distance runner. Medical researchers have now shown that during endurance exercise the protein PGC-1alpha shifts the metabolic profile in the muscle.

Are There Evidence-Based Dietary Interventions for Multiple Sclerosis?

Multiple sclerosis (MS) is a disease whose precise origin is unknown, although it is generally accepted that MS involves an immune-mediated process. The disease is characterized by the destruction of myelin sheath, the protective material that surrounds and protects nerve cells in the brain and spinal cord. The progressive loss and thinning of myelin leads to the formation of scar tissue or plaques. This interferes with the transmission of nerve signals producing symptoms which vary depending on where damage has occurred. Common symptoms of MS include fatigue, pain, numbness, blurred vision, cognitive impairment, and bladder and bowel dysfunction, but the course of MS is unpredictable and varies individually, with some patients experiencing minimal impairment. There is no cure for MS. Medi...
