How women can stop cycling from being a pain in the arse | Dawn Foster

It will be a problem familiar to many female cyclists. There's no one-size-fits-all solution ? it's about learning what works for you

Women risk a loss of sensation, numbness and tingling in their undercarriage as a result of cycling in certain positions, the New York Times reports. In a study published in the Journal of Sexual Medicine, Yale academics asked 48 competitive cyclists to bring their bikes into their laboratory, then used a device to test sensation in the pelvic floor. They found that cycling in the drop position increased perineum pain, and decreased genital sensation. Boston University previously found that cycling contributes to erectile dysfunction, so the lads don't get an easy ride either.

It's a finding that will be familiar to many female cyclists. Days into cycling, I walked into the pub like John Wayne and sat down gingerly. My friend, a seasoned cyclist, put her hand on my shoulder, gave me a knowing look and reassured me: "It gets better." It did of course. And a large part of cycling is learning what works for you.

The solutions are often counter-intuitive: a gel saddle is likely to cause more discomfort than a hard Brooks saddle, for instance, because the material bunches up, causing more friction. You'll need to "break in" the saddle first, if you're choosing one with minimal padding, but leather saddles stretch and mould to fit your backside perfectly.

Adjusting handlebars can make all the difference, changing the amount of weight you put on your arms: lowering the stem can redistribute some of the pressure to your arms, as can raising, or lowering the saddle a little. Playing around with saddle positions can help you figure out what's best for you ? there's no one-size-fits-all solution.

The bike you choose will have a strong bearing on your saddle choice. If you're on an upright bike, the width of your saddle will be an issue. Unisex and men's saddles are typically narrower, which can be a literal pain in the arse for women. Women's pelvises are wider, so wider saddles will support your sit bones more adequately. With all the weight then distributed evenly, you shouldn't have any problems with support and comfort.

Racing and touring bikes, especially those with drop handles, have a different issue, and it was this type of bike that was highlighted as problematic by the study. When cycling in the drop position, your weight is focused on your clitoral region. After a few hours of cycling, this can cause pain and numbness. The noses on women's saddles tend to be a tad shorter than men's for this reason ? often tilting the nose a tiny bit forward can make the world of difference. Cutouts in saddles can offer a bit of relief and are a common feature on women's racing saddles. Switching between resting on the hoods (the stationary part of the brakes), and riding in the drop position can also give your delicates a break on long rides, too.

It's not all saddles though. Especially in hot weather and on long cycle rides, chamois cream can ease intimate friction and after long rides, Sudocrem is a godsend for sensitive areas. Padded cycling shorts are also a great investment - you can get hot-pant style shorts that are handily hidden under skirts for city cycling, and they also temper worries about inadvertent underwear flashing as you're whizzing down the road. On long rides, if you're wearing padded cycling tights, the best underwear choice is the most liberating cycling secret of all ? go commando. Cyclists of Britain, cast off your knickers - you have nothing to lose but the danger of some chafing.

A straw poll of friends showed that discomfort when cycling was a very common problem, but not one that most women felt comfortable bringing up. A few people reported the opposite effect ? hypersensitivity. In which chase, grab your tandems, and enjoy the summer sun.

Steel City downhill ? the race that brings together cycling and wildlife | Ed Douglas

Former world champion Steve Peat backs a new mountain bike trail that provides the thrill but doesn't damage nature

I'm leaning against a pine tree in Grenoside Woods on the outskirts of Sheffield, watching local hero Steve Peat flash by on his mountain bike. Peaty, as everyone seems to call him, is one of the all-time greats of downhill racing, world champion in 2009 and a world cup champion three times over.

Today's race may not be in that league, but it's special nonetheless. Peat was born just down the hill in Chapeltown, and when he was learning his craft, he'd cycle up through these woods to reach the steeper downhill trails at Wharncliffe. This is home ground.

But that's not the only reason he's offered his backing to the Steel City downhill event. The race is also a fundraiser for the Sheffield Wildlife Trust, which is completing a 1m purchase of the 440-acre site both for wildlife and the people of Sheffield.

At the finish line, a sound system is belting out the Red Hot Chilli Peppers, and young lads and a lasses are knocking back freebie caffeine drinks while leaning on some very pricey bicycles. They don't look like typical nature conservation types, but as Peat explains, looks can be deceptive.

"I've been riding through Greno for 20 years, so it's great for me to help out SWT and help them raise funds to secure the woods," he says. The new trail he's racing on will help divert recreational riders away from the more fragile area of heath land just beyond the trees. "We're happy to tell other riders that a few trails have been moved away from the heath so SWT can fence it off and start grazing cattle there."

Sheffield has a huge mountain biking scene, supporting several specialist shops and manufacturers, including cult frame-makers Cotic. But there are proportionally fewer bridleways than most other parts of the country, and frustrated riders have been at loggerheads with the city council and other users, including some walkers and horse riders.

Although the Trans Pennine trail goes through Grenoside, there are few designated bridleways in the woods and manicured surfaces aren't what riders like Peat want. They like their tracks a lot more challenging. But negotiating access to the countryside has been outside the scope of existing cycling groups, which campaign on safety and planning issues, or else oversee competition.

So in February 2010, a group of local riders met in a pub to launch Ride Sheffield, an advocacy group aimed at fighting mountain biking's corner to improve access. The group was the brainchild of Henry Norman, whose day job is working for cycling charity Sustrans.

"We wanted to give mountain biking a voice," he says, adding that mountain bikers are less likely to join a club or organise in the way walkers and climbers do. "There were consultations going on about local rights of way, but there was no one for the council or other agencies to talk to about our interests."

Ride Sheffield has around 700 bikers signed up. It costs nothing to join, and relies on volunteers to attend meetings where access is up for discussion. Despite limited resources, it has quickly proved itself an effective voice. Apart from Grenoside, there are schemes and campaigns across the city to improve access and build new trails. Most recently, the National Trust and RSPB, which jointly manage the wild moors southwest of Sheffield, have agreed to create permissive bridleways on a few carefully chosen footpaths.

Roy Mosley of Sheffield Wildlife Trust says that with a largely urban portfolio of properties, his organisation has to be pragmatic. "It comes with having sites on people's doorsteps. We want them to come, so we try to manage things to support biodiversity." Liaising with Ride Sheffield has, he says, produced a more interesting bike trail in a less sensitive part of the woods.

"Ride Sheffield has made things happen," he says, "especially in influencing the city council. There'll always one or two riders who don't behave, but that's true for any activity."

Cycle stunt team launches competition to find new female member | Kirsty Scott

Scottish team The Clan is looking for a committed and fearless rider to 'show all the girls out there riding is not just for boys'

Scotland's cycle stunt team, The Clan, have launched a competition to find a new member to join them on this summer's tour of events. The winning rider must be skilled, committed, fearless and, most importantly of all, female.

It's the second year in a row that The Clan, the only UK stunt cycling team to feature female riders, has run a competition to find a new female member. Last year, they had six entrants. This year, with more than a week still to go before the deadline, they already have 10 in the running for the 5,000 prize; a sign, say the organisers, of a growing interest in the sport, and cycling in general, among girls and women.

Since they formed in 2008 ? Danny MacAskill was a founder member ? The Clan have insisted on having female members for the majority of their shows, relying initially on recommendations from friends and acquaintances of the six-strong team.

The Clan manager, Iain Withers, says:

"It's important to have diversity in our team and especially to ensure a female rider is present to show all the girls out there riding is not just for boys. "It comes from the vibe of our show, trying to convince people that cycling is cool and fun and exciting for everyone. We have employed female riders since year one. We were going round schools for Cycling Scotland and there was a lot of research that made it quite clear that if girls are going to get into cycling they need female role models to look up to. That's what started it off.

"We were a little bit daunted to go and find a female rider because it's such a male dominated area. We didn't know if there was anyone out there who would be willing and capable but we have been really lucky. There have been six girls who have now done our shows and really the lesson has been that the talent is out there but it is just hidden away This year we have already got 10 really strong entries, quite a diverse range of talent from BMX to mountain biking. It's very promising."

The group runs shows at national sports events and community galas across Scotland in May and June. This year, for the first time, they'll be holding a women's weekend for all competition entrants in early May so that they can train and practice together on custom-built ramps and in a foam pit designed to allow riders to crash without hurting themselves.

Age, says Withers, should be no barrier.

"Last year, we had a very strong entry from a mountain biker who was 15 at the time. She can't do it this year because she has exams but she's a really talented rider. Our first rider, Lynne Aitchison, who has since moved to Canada, is in her mid-30s. You can be any age. If we get some granny flying through the air that would be good stuff."

Speaking from Whistler in British Columbia, Aitchison says The Clan's insistence on featuring female riders is crucial for promoting the sport as widely as possible.

"Having a female member of The Clan has been a really good thing as far as getting the message across to young girls that it's not just about boys," she says. "When we went round schools the girls thought I was the bee's knees because I was a girl and I was doing all the tricks as well."

Aitchison only got into cycling in her late 20s and was in her 30s when she joined The Clan.

"I did martial arts for a long time but I got a bit sick of doing sports inside. I had a bunch of male friends who mountain biked and I thought I'd try it out and I was hooked. I found I kind of excelled at jumping. I did my first back flip when I was 31.

"It was a little intimidating for me at first to ride with a bunch of pretty highly skilled guys. I was 30 years old and I didn't get into biking until my late 20s. The guys in The Clan are really down to earth, really nice guys. They're all about riding rather than the kudos of being a bit of a stunt cyclist."

Aitchison has also set up Air Maiden the annual women-only bike skills event at Glentress in the Tweed Valley. She had noticed that she was often the only female at the freeride skills park despite the fact that more women were getting into mountain biking.

"It's a really comfortable environment," she says. "There are no guys there to intimidate the women. It's really encouraging of them and it's grown so much that we're turning them away."

Aitchison will be back in Scotland for The Clan's women-only weekend in May and to offer support and encouragement to the competition winner. As much as she might have wanted to be involved, there was no chance of her being able to enter the competition this year.

"I have a seven-month old son and I'm pregnant with twins so I'm not going to be riding my bike for a while."

From feast to famine: A metabolic switch that may help diabetes treatment

Humans are built to hunger for fat, packing it on during times of feast and burning it during periods of famine. But when deluged by foods rich in fat and sugar, the modern waistline often far exceeds the need to store energy for lean times, and the result has been an epidemic of diabetes, heart disease and other obesity-related problems. Now, scientists have identified the linchpin of fat metabolism, a protein known as fibroblast growth factor 1 (FGF1), which may open new avenues in the treatment of diabetes.

Prevalence of metabolically healthy obesity and its impacts on incidences of hypertension, diabetes and the metabolic syndrome in Taiwan.

This study examined the prevalence of baseline metabolically healthy obese subjects and its impacts on the incidences of cardiometabolic diseases using a nation-wide population cohort. Metabolically healthy obese were prevalent in 8.2% of the baseline population and 28.5% of the obese subjects. Subjects included were 1,547 men and women (age range, 18-59 years), who were free of components of the metabolic syndrome except waist criteria. During an average 5.4-year follow-up, the cumulative incidences of hypertension, type 2 diabetes and the metabolic syndrome were 7.8%, 1.2% and 5.6%, respectively. The hazard ratios (95% CIs) for the metabolic syndrome incidence were significantly higher at BMI levels of >=23.0 kg/m2 [4.68 (2.22-9.86)] for BMI of 23-24.9 kg/m2; 8.82 (4.01-19.4) for BMI ...

Faecal calprotectin - a useful tool in the management of inflammatory bowel disease.

Authors: Burri E, Beglinger C
Abstract
Inflammatory bowel disease (IBD) should be suspected in any patient presenting with chronic or recurrent abdominal pain and diarrhoea. Current guidelines suggest performing invasive endoscopy with histological sampling for further diagnosis. Measuring calprotectin, a neutrophilic protein, in faeces has been proposed as a surrogate marker of intestinal inflammation. Calprotectin values have been shown to reliably differentiate between IBD and non-organic disease in symptomatic patients and, when elevated, warrant early endoscopic investigation to rule out IBD and other organic pathologies. Endoscopy with histological sampling is also used to evaluate disease activity and here, too, faecal calprotectin values seem to correlate well. In a number ...

Lumenal lipid metabolism: implications for lipoprotein assembly.

Authors: Lehner R, Lian J, Quiroga AD
Abstract
Overproduction of apolipoprotein B (apoB)-containing lipoproteins by the liver and the intestine is 1 of the hallmarks of insulin resistance and type 2 diabetes and a well-established risk factor of cardiovascular disease. The assembly of apoB lipoproteins is regulated by the availability of lipids that form the neutral lipid core (triacylglycerol and cholesteryl ester) and the limiting lipoprotein monolayer (phospholipids and cholesterol). Although tremendous advances have been made over the past decade toward understanding neutral lipid and phospholipid biosynthesis and neutral lipid storage in cytosolic lipid droplets (LDs), little is known about the mechanisms that govern the transfer of lipids to the lumen of the endoplasmic retic...

Markers of Sleep-Disordered Breathing and Metabolic Syndrome in a Multiethnic Sample of US Adults: Results from the National Health and Nutrition Examination Survey 2005–2008

We examined 6,122 participants aged ≥20 years from the National Health and Nutrition Examination Survey 2005–08. Metabolic syndrome was defined as the presence of ≥3 of the following components: (1) abdominal obesity, (2) elevated blood triglycerides, (3) low HDL cholesterol, (4) high BP, and (5) hyperglycemia. SDB severity was defined based on an additive summary score including sleep duration, snoring, snorting, and daytime sleepiness. We found that short sleep duration, snoring, snorting, daytime sleepiness and the summary SDB score were significantly associated with metabolic syndrome independent of potential confounders. Compared to those without any sleep disturbance, the multivariable odds ratio (OR) (95% confidence interval [CI]) of metabolic syndrome among th...

Risky communication in atherosclerosis and thrombus formation.

Authors: Meens M, Pfenniger A, Kwak B
Abstract
Atherosclerosis, a progressive disease of medium- and large-sized arteries, constitutes the major cause of death in developed countries, and is becoming increasingly prevalent in developing countries as well. The main consequences of atherosclerosis are myocardial infarction, cerebral infarction and aortic aneurysm. This inflammatory disease is characterised by specific intimal lesions where lipids, leukocytes and smooth muscle cells accumulate in the arterial wall over time. Risk factors for atherosclerosis can mainly be divided into two groups: i) risk factors induced by environment and behaviour (e.g., Western diet, smoking and sedentary lifestyle) and ii) genetic risk factors. Multiple epidemiological studies have associated a sing...

The development of a global program for the elimination of brain damage due to iodine deficiency.

Authors: Hetzel BS
Abstract
Iodine deficiency is the most common preventable cause of brain damage with more than 2 billion people from 130 countries at risk. Runner's high motivated the evolution of exercise, research suggests

Runners often extol the virtues of the runner's high, but now a team of researchers suggest that the runner's high could have evolved to motivate us to exercise as part of our early long-distance nomadic lifestyle.

Motivation to exercise affects behavior

For many people, the motivation to exercise fluctuates from week to week, and these fluctuations predict whether they will be physically active, according to researchers. In an effort to understand how the motivation to exercise is linked to behavior, the researchers examined college students' intentions to be physically active as well as their actual activity levels.

The development of a global program for the elimination of brain damage due to iodine deficiency.

Authors: Hetzel BS
Abstract
Iodine deficiency is the most common preventable cause of brain damage with more than 2 billion people from 130 countries at risk. The global problem of iodine deficiency has been redefined by a readily transmitted population concept, with an easy acronym - the concept of the iodine deficiency disorders (IDD) - referring to all the effects of iodine deficiency in a population, that can be totally prevented by correction of iodine deficiency with special emphasis on brain damage and not just to goitre and cretinism (1983). This was followed by the creation of the International Council for Control of Iodine Deficiency Disorders (ICCIDD) supported by WHO and UNICEF with 700 multidisciplinary professionals from more than 100 countries, committed to providing...

Bringing care closer to home

Meet Kevin Blake, a long-time patient of Children?s. In the following, his mother, Laurie, describes what having access to Children?s services closer to home has meant to their family.

Kevin and brother David

My son Kevin was born with multiple congenital abnormalities. He has moderate to severe hearing loss, low muscle tone and a handful of developmental delays. When he was born we instantly knew he was a special kid, and I immediately felt very protective of him. Life may be a bit tougher for Kevin, but his vulnerabilities and strength to work through them is nothing short of amazing, and in doing so he inspires my husband Dan and I to be better parents. As Dan says, ?If there are a certain percentage of children born with special needs, then I?m glad we have one of them, be...

How poor maternal diet can increase risk of diabetes: New mechanism discovered

Researchers have shown one way in which poor nutrition in the womb can put a person at greater risk of developing type 2 diabetes and other age-related diseases in later life. This finding could lead to new ways of identifying people who are at a higher risk of developing these diseases and might open up targets for treatment.

Mice with an extra dose of a known anti-cancer gene lose weight even as their appetites grow. Not only that, but according to new research, the animals also live longer, and that isn't just because they aren't getting cancer, either.

Pennsylvania Law To End Mandatory Overtime for Some Health Care Workers

Pennsylvania Gov. Ed Rendell (D) on Thursday signed into law a bill that will prohibit health care facilities from requiring mandatory overtime of staff, the Philadelphia Inquirer reports. The law, set to take effect in July 2009, seeks to protect health care facilities from forcing nurses, technicians, technologists, certified nursing assistants and phlebotomists, and other caregivers from being disciplined or discriminated against for refusing to work overtime. Mandatory overtime would be allowed in some cases, such as a natural disaster, but it could be used only as a last resort and employees would have to be given one hour to arrange for family care. Employers also are prohibited from using on-call time in place of mandatory overtime. Violators can be fined from $100 to $1,000 per vio...

How Touching Saves Lives

When I was a fourth-year medical student, I once did a month-long rotation in the ER. One night a woman came in who we decided needed some lab work.
