Hot Topics in Tetralogy of Fallot

Tetralogy of Fallot (TOF) is the most common cyanotic congenital heart defect. We explore ?hot topics? to highlight areas of emerging science for clinicians and scientists in moving toward a better understanding of the long-term management of patients with repaired TOF. From a genetic perspective, the etiology of TOF is multifactorial, with a familial recurrence risk of 3%. Cardiac magnetic resonance is the gold standard assessment tool based on its superior imaging of the right ventricular (RV) outflow tract, pulmonary arteries, aorta, and aortopulmonary collaterals, and on its ability to quantify biventricular size and function, pulmonary regurgitation (PR), and myocardial viability. Atrial re-entrant tachycardia will develop in more than 30% of patients, and high-grade ventricular a...

'Cannibal sandwiches' sicken residents in Midwest

(Reuters) - Residents in the upper Midwest should ditch their seasonal tradition of eating "cannibal sandwiches" made of raw ground beef, health officials warned, citing multiple outbreaks of foodborne illnesses since the 1970s and cases last year.

The Shell Centre closure means losing a pearl of a swimming pool

I'm a keen campaigner to keep public pools open; now I've surprised myself by caring, too, when a corporate one is shut down. The Shell pool on London's South Bank will be a sad loss

Another pool down.

I've been to many pools around London in the course of writing a book about the city's best swimming places. Some of them are excellent, some slightly harder to love. But while I'm not completely below stairs ? more hovering on the bottom few steps looking apologetic ? there are a few that the likes of me won't ever get into. Believe me, I've tried; I've knocked on the doors of pretty much every private establishment of note. "I bet if I was Pippa Middleton you'd let me in," I grumbled to one receptionist. "Not even Pippa Middleton would get in if she wasn't a member or guest of a member," she said, seeing my grump and raising it with snoot. Imagine if Pippa had heard that. She'd have been devastated.

On top of all these posh places I'm excluded from (I say "I'm" because you could be members of the Hurlingham Club for all I know), there's another lot: the company pool only open to employees. I imagine these to be similar to the private gym pools that proliferate anywhere there's someone making money. They're usually the size of a big sink and less concerned with the quality of the swim than with the peripheral niceties ? those desirable little waffle slippers; free product in the showers not just because someone else forgot to pick theirs up. I'm committed to my swimming, but it seemed excessive to me, getting a job with a company simply to have a go in their pool. Particularly if that company was Shell. I tried being corporate once, I wasn't very good at it, and even on a good day I'm not exactly a Shell kind of person. Not that they're crawling over themselves to get at someone with my, let's call it, "portfolio" CV.

You'd never know to look at it, but the Shell Centre on London's South Bank includes in its basement a rather fabulous full-sized pool. Built in 1961, it's open to employees (natch), and a few local groups use it too, though you need more paperwork than for emigrating to Australia. Far below the clippy-cloppy world of corporate suits and heels, beyond the cool marble of the modern reception area, is a journey back in time. To get to the pool (or the staff rifle range, should that be more your thing), you go in lifts and through doors and down stairs of coloured heavy-duty lino; you get a glimpse of your destination through a slatted blind of a window high above. You continue along acres of concrete and corridors lined with dark-varnished 60s plywood, then you pass massive indoor sports halls and gyms until you're deep in the bowels of the building. Maybe you're nearly under the actual Thames by now ? exciting! It's like being in Antz.

What you get when you eventually arrive is an architectural and interior-design gem, mostly untouched since it was built, and pristinely maintained. The changing rooms are film-set perfect; there's an old "powder room" for post-swim hair and makeup and blocks of interlinked cubicles made of granite (or more probably concrete). The showers are newer ? the horrid colour scheme screams 80s, and looks so brash next to the pastel originals. The loos have a machine dispensing disposable toilet seat covers; you can really picture the place buzzing with neat secretarial staff, all set-hair and 60s skirt-suits. You go through a now-dry footbath, and two shower curtains providing a small bit of privacy, to the pool itself. Wow. The hall is huge; the spectator gallery along one side has the kind of plywood seats that have become fashionable again ? these are originals. Everything is period, even down to the timing clock. The ceiling is a high slab of square tiles (probably asbestos); the walls are tiled mushroom and yellow to just above head height, and plain Artex above that. The whole thing feels so meticulous, you start talking with that very particular clipped British information film voice. Careful, Miss Landreth, not to get your hair wet! You have to take notes for that meeting at three!

The pool is great. Very solid, proportionally perfect, it's 33m long with three original diving boards and a proper deep end. It's deep throughout, actually, which tells you it predates the whole concept of "leisure centre". The pool has a beautifully kept mosaic-tiled bottom, and the wall behind the diving boards is tiled a glossy dark blue-green, almost with the depth of colour of swirling oil; there's a motif ? a shell ? in the top left hand corner. It's so very stylish, it's Mad Men in swimming pool form. It's one of the best, purest examples of 1960s design in London. An utter delight ? and it's going.

You'll know by now that I'm a keen campaigner to keep public pools open. So I've surprised myself by caring, too, when I hear about a corporate pool being demolished. The Shell building has been sold and the whole lot is going to be developed, including the pool. It'll be a car park, or house all the building gubbins, the machinery and that, who cares what. I just care that it's going. I know the pool was never "ours" to start with, so it's not "ours" to lose, but nonetheless it feels like a terrible shame. There's talk of a new pool, further up in Coin St, which will be public. So in a sense, we gain from this. But I can tell you now, it won't have a quarter of the charm or elegance of the one here. It'll be an oversized sink, and we'll be grateful.

I wanted to save it; I went so far as to look at the English Heritage procedure for listing buildings, but realised it was way outside the capabilities and remit of a noisy idiot like me. I don't think even Pippa Middleton could click her fingers and say, "Here, someone, save this" to any effect. It's all too late, even for Pippa; it's going under the wrecking ball in the next month. And that is a massive, massive shame.

Oh, and the staff rifle range. Don't feel quite so passionate about that. Someone else can take up that particular cause.

Swimming lessons: the Miracle Swimming method

Designed to give confidence to absolute beginners, Miracle Swimming aims, first of all, to put you at ease in the water

What swimming method have you been trying this time?
I had a go at the Miracle Swimming method with A2Z Swim this week.

That's a bold name. What's so miraculous about it?
In short, it teaches you not to be afraid of the water. Which is difficult when you think about it, because instinctively and for good reason we're afraid of drowning.

After all these lessons I'd have thought you'd be fearless by now ?
Funnily enough, I'm not. I do have to psych myself up to climb into the water, though I am getting used to it. This method starts at the very very very beginning, before you even start to put together the building blocks of strokes, and concentrates on teaching you to be confident and safe in the water first.

Sounds like it makes sense. Who came up with the idea?
It was created by M Ellen (Melon) Dash, an American competitive swimmer. She was teaching undergraduate learners at Keene State College in New Hampshire in the 1970s and gave them instructions on how to do flutter kick. When they all stared at her, terrified, she realised that it was pointless telling people what to do with their arms and legs when they were afraid they might not live. She worked out a way to get people over their fears first.

Reading on mobile? Click here to view

How does it work?
The founding principle is that when you feel completely comfortable you're in the best condition to learn. You're present in your body and not scared. There are five stages ? at ease, cold feet, fluttery tummy, heart in your mouth, and panic. These are called the Five Circles, and the idea is that you always stay in the first circle as you learn. It makes it a very gentle method that only goes as fast as you are ready to.

What happened in the lesson you had?
We started outside of the pool and Andrea and Zoe (the A and Z in the A2Z name) explained how they were going to teach us ? that we had to make sure we were always in our first circle, and that we wouldn't race towards learning strokes. They emphasised that it was more important that we felt safe and trusted the water to hold us up than to be able to move through it. Learning to swim is like a Jenga tower, they said. You can build it up quite fast, but if you've not got all the blocks in place at every level the tower will eventually collapse. We got into a very shallow teaching pool, no more than a metre deep, and lovely and warm, which took away some of the shock factor about getting into water. We spent time acclimatising to the feel of the water, lowering in slowly, floating, moving our arms through the water and then playing some games knocking down diving sticks to get a feel for how the water moves.

Reading on mobile? Click here to view

So no actual swimming?
If you mean powering up and down the pool doing freestyle, then no, not in this first lesson. If you define swimming as being safe and confident in water, as they do, then yes. It was good to not have the pressure of having to achieve and to just be in the water. I was able to take the time to see where the tension is in my body, how relaxed I felt or not, and how the water actually feels. It made me realise how much progress I'd made in not feeling afraid, but also that I wasn't as fearless as I imagined.

Where can I have a go?
A2ZSwim operate mostly between Cheltenham and Oxford, but are planning on running courses in other parts of the country. You can take an introductory class for three hours, or check out their beginner class, which is 24 hours of classes run over eight sessions. They also run Next Step courses, 3:1 sessions, family sessions and bespoke classes.

Who's it good for?
Well in theory, everybody! Even if you feel you can swim, if you prefer not to put your face in the water they'd probably argue that you have an unaddressed fear ? a missing block in your Jenga tower ? that means you're not completely relaxed in the pool. I keep learning that being relaxed is the key to being able to float well and move well through the water, so it makes sense to go right back to the start and fill in the gaps. If you're absolutely terrified, this method is perfect. It's not pushy or insensitive ? it's designed for people who are scared, and won't make you feel silly for being afraid.

Health care providers should aggressively treat unhealthy lifestyles

Unhealthy habits, such as smoking, poor diet, and being overweight should be treated as aggressively as high blood pressure, high cholesterol and other cardiovascular disease risk factors. Insurance reimbursement policies need to be improved so that registered dietitians, psychologists and others can become part of the primary practice team.

Bringing care closer to home

Meet Kevin Blake, a long-time patient of Children?s. In the following, his mother, Laurie, describes what having access to Children?s services closer to home has meant to their family.

Kevin and brother David

My son Kevin was born with multiple congenital abnormalities. He has moderate to severe hearing loss, low muscle tone and a handful of developmental delays. When he was born we instantly knew he was a special kid, and I immediately felt very protective of him. Life may be a bit tougher for Kevin, but his vulnerabilities and strength to work through them is nothing short of amazing, and in doing so he inspires my husband Dan and I to be better parents. As Dan says, ?If there are a certain percentage of children born with special needs, then I?m glad we have one of them, be...

Streak for Christmas (with your kit on)

Why not join the Advent Streak and run every day in December up to Christmas Day?

How do you fancy becoming a streaker this Christmas season? Don't worry, you can keep your clothes on. Your running clothes, of course. In fact, there will be little point in taking them off. This is because, to join us in our streak, you will need to run every day from 1 December to Christmas Day. It is called the Advent Streak ? and any chocolate in your advent calendar can only be consumed once you have completed that day's run. Anyone up for the challenge?

The Advent Streak was an idea initiated by the online running group Run Mummy Run, and there are now about 50 people taking part. The idea is that you choose a minimum distance, such as a mile or 5km, and run at least that distance each day for the 25 days. If you do it smartly, using what would normally be your rest days as easy recovery runs of only your minimum distance, then you should find that your fitness improves.

Of course, streaking is not new. Mark Covert ran every day for 45 years until this year, when a foot injury sadly caused his retirement. And then there's former Commonwealth marathon champion Ron Hill, who maintained his streak by running a mile the day after breaking his sternum in car crash. Many others sign up for a month's streak, such as Janathon.

The theory behind streaking is that, if you are used to exercise, your body can partake in "active recovery". Instead of taking complete rest (and, let's face it, how many of us truly do rest on our rest days anyway?), your tired muscles can be allowed to recover through easier work.

My own interest in streaking began in April this year, when I ran at least 5km per day for 50 days. I started it a week after running the Brighton Marathon as a way to keep up my fitness, rather than let myself sink into my usual post-marathon training low. My running improved so much that I soon smashed my PBs for 5k and 10k.

Running every day also led me to seek out new routes, take on more off-road work, and try new local running groups. I relished the three days per week where, after choosing a route of roughly 5km, I could leave my watch at home and just run nice and easy, giving me much-needed time to myself.

Though a mere 25 days, I'm certain the Advent Streak will bring some of the same benefits. If you are preparing for spring distance training, this will give you a fantastic kickstart. But even if you aren't, the streak will give you the motivation we all sometimes need to get out there on cold winter mornings or evenings. It may even help keep off some of the pounds that many of us gain at this time of year.

There was much discussion among members of Run Mummy Run as to how we can find the time to run every day. But with a few adjustments, almost anybody can do it. Many members will be running with their children in their buggies. One amazing lady will be doing some of her miles while pushing her son in his wheelchair, and one other will have to do one of her runs in an airport. Whatever your circumstances, you can make the time.

If you do choose to take part, there are some precautions you should take too. Firstly, as already stated, you should not be pushing yourself every single day. On the days that are ordinarily your recovery days, just run your minimum distance at a very easy, conversation pace.

I you are new or returning to running, or are recovering from illness or injury, the streak is definitely not the time to start building your mileage over any single run. And if an injury rears its ugly head, do not ignore it. Remember this is just a challenge for fun and fitness, and it should not result in your becoming unable to run afterwards.

You may be running in the dark more than usual, so do take all necessary precautions to keep yourself safe. Wherever possible, run with others. Finally, even if time is short, do carry out at least a few basic stretches after your run.

So are you going to join us? You can join the Facebook event and we would love to hear from you about how the streak is going. So go streak and be merry!

Pathophysiology of Anemia During the Neonatal Period, Including Anemia of Prematurity

This review summarizes the current thinking about the causes of anemia universally experienced by preterm infants in the early postnatal weeks. In addition to describing developmentally determined physiologic processes contributing to anemia of prematurity, this review discusses clinically important nonphysiologic contributors to anemia experienced by preterm infants during the neonatal period. Chief among these and an important contributor to the need for red blood cell transfusions is the heavy laboratory phlebotomy loss sustained shortly after birth, when neonatal cardiorespiratory illness is most severe. Understanding and recognizing the physiologic and nonphysiologic processes contributing to anemia encountered in early postnatal life is important in knowing which treatment and preven...

Heroes of swimming: Johnny Weissmuller

Before he became ridiculously famous as Tarzan, today's hero broke world records for fun ? and was undefeated in 10 years of competition swimming

"So the producer asked my name and he said it would never do. 'It's too long. It'll never fit on a marquee. We'll have to change it.' Then the director butted in: 'Don't you ever read the papers? This guy is the world's greatest swimmer.' The producer said OK, I could keep my name, and he told the writers, 'Put a lot of swimming in the movie, because this guy can swim.'"

The guy ? whose name was Johnny Weissmuller, and who had just been given the role of Tarzan ? could definitely swim. In fact, "the world's greatest swimmer" was true then, and it's probably still true today. To get a measure of just how good he was, try this single fact for size: in 10 years of competition swimming, Johnny Weissmuller never lost a race. Not once.

No one has ever come close to a winning streak like Weissmuller's, certainly not in swimming. Michael Phelps spent a decade undefeated at 200m butterfly, but lost in other disciplines during that time. Tams Darnyi of Hungary probably came closest, with an eight-year undefeated streak in the brutal 200m and 400m medley events. But Weissmuller raced distances ranging from 50m to half a mile. He swam mostly front crawl, but also set world records for backstroke ("I got bored," he said, "so I swam on my back, where I could spend more time looking around.")

This incredible undefeated period made Weissmuller part of what US sportswriters dubbed "the golden age". The 1920s was a time when America seemed to be producing a string of world-beaters, and Johnny Weissmuller was right up there alongside superstars such as Babe Ruth, Bill Tilden and Jack Dempsey.

Johnny broke world records for fun: at swim meets, in demonstrations, in Olympic finals, at national championships, even in the training pool in front of just his coach, a lifeguard and a cleaner. He got canny about it: his coach had promised to buy him lunch when he broke a record, so Johnny started shaving off a few hundredths here, a tenth there, always making sure there was more in the tank for next time he was hungry.

Officially, by the time he retired Johnny had broken 67 world records. Swimming buffs, though, still repeat rumours that Weissmuller actually set new world marks hundreds of times and just didn't bother to submit the paperwork. He won five Olympic gold medals for swimming, and one bronze, for water polo:

"Water polo's a rough game. We never could beat those Yugoslavians. [The referee] never blows the whistle over there. Anyhow, that's where I learned to duck. It came in handy later, when Cheeta started throwing coconuts."

When he retired from competition swimming, Weissmuller spent time as a beach bum in Florida, then worked as a swimsuit model, touring the country. At 6ft 2 and 86kg, he cut an imposing figure in a pair of budgie smugglers; in retrospect it's not surprising that in 1931, when Hollywood was looking for a new Tarzan, Johnny got a call. He admitted later:

"It was like stealing. There was swimming in it, and I didn't have much to say. How can a guy climb trees, say 'Me Tarzan, you Jane', and make a million?"

Whatever you think of his acting skills, Johnny took to Hollywood like a duck to water. He became such a big star that he didn't have to be a good actor. He made six Tarzan movies with MGM, considered to by your correspondent to be the pinnacle of the genre. He made six more for RKO, which aren't as good. He then swapped pretend Africa for pretend Asia, donned a safari suit and began playing Jungle Jim the hunter. Weissmuller made 13 Jungle Jim films between 1948 and 1954 ? but there's not much swimming in them, and today they're largely unremembered.

By the 1950s, Johnny had made millions in Hollywood. He'd been married four times (with one more wife to come, in 1963), including a tempestuous espousal to the actress Lupe Vlez, "the Mexican Spitfire". That marriage provided gossip columnists with five years of public and private brawling, shouting matches, splitting up and making up, until it ended In 1948. Weissmuller's fame far transcended his swimming achievements, and continues to do so. How many other swimming stars are there on the cover of Sergeant Pepper's Lonely Heart Club Band, after all?

There are many stories about Johnny, but one of my favourites is set against the backdrop of the Cuban revolution. Weissmuller was a strong amateur golfer, and in 1958 he was in Cuba playing in a celebrity tournament. Suddenly, men with guns ? part of the rebel army ? appeared on the course. Things seemed about to get a bit hairy. But Johnny climbed out of his golf cart and gave his trademark yell: "Aaaaaah, ah-ya-ah-ah aaaaaaaah!" "Tarzan!" a voice replied. "Welcome to Cuba!" Hands were shaken, and the rebels escorted the Americans back to the clubhouse.

In 1974, Weissmuller fell and broke his hip and leg. In hospital, doctors discovered he had a lifelong heart condition, which in the coming years finally began to tell on him. In 1977 Johnny had a series of strokes, and in 1984 he died in Acapulco, Mexico. He's buried there, at the Valley of the Light cemetery.
